Homes for Hope, Inc.’s Housing Program mission:
is to provide permanent housing and to support residents as they receive comprehensive services to initiate and promote the transition from homelessness to productivity and independence. The program intent is to assist the residents with problem-solving and encourage participation in activities to support themselves to maintain independence. This innovative method has proven successful in helping extremely vulnerable and hard-to-reach populations to stabilize their lives and move forward to independence with dignity.
The Homes for Hope, Inc.’s Housing Program:
provides referrals for permanent scattered housing sites throughout the District of Columbia to homeless individuals who may be diagnosed with chronic illnesses, substance abuse history, and/or mental health challenges. In addition, we provide client centered supportive services to include, but, are not limited to, intensive case management, substance abuse and mental health support groups and linkages and referrals to service providers.
For some people intensive support could be “time limited.” For others, it may be “life-long. “
How do you qualify?
Eligibility requirements:
- Homeless persons or persons at risk for homelessness (who occupy emergency shelters, short-term transitional housing or sleeping on the streets).
- Challenged with chronic illness.
- Ability to meet the HFH, Inc. financial criteria for lease based units.
- Participants must agree to participate in the following supportive services (which vary on an individual basis):
